This may be in addition to anything else you want to submit to the event.

Which is the Best Form to Use?

It doesn't matter which you choose. They all pop into the same inbox.  Go for the one most appropriate for yourself.

What's the Deadline?

Last day of the month, if you wish it to be included in the Death Note Month of... event.

However, we're unlikely to turn down suitable content* at any other time either. It just won't be listed in the round-up of each topic's Month of... articles beyond that final date on the calendar.
* Please keep submissions family friendly/appropriate for all ages.  That describes the Death Note News readership anyway, insofar as we can tell.
